We offer various types of round-the-clock support, including in-home or centre-based support, holiday-based support, supported for selected days, and emergency support.

Support Coordination and Specialist Support Coordination

AFFIRMING FUTURES has qualified support coordinators that will be there to help you understand your NDIS plan and make the most out of it. AFFIRMING FUTURES will support you to build your ability to connect to informal, community, and funded supports (mainstream and other) to achieve your goals.

AFFIRMING FUTURES will work with you to build the skills you need to understand, implement, and use your plan. AFFIRMING FUTURES support coordinators will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.

AFFIRMING FUTURES have qualified social workers and specialist support coordinators with over 20 years of experience in disability work who are able to provide a higher level of complex support coordination. Our specialist support coordination aims to reduce any barriers to the full implementation and use of your plan.

Specialist Behaviour Support

AFFIRMING FUTURES have experienced behaviour support practitioners that would support participants with complex and challenging behaviours. We use holistic behavioural functional assessments to design plans for participants with a goal of increasing their quality of life and minimising restrictive practices. We will work with participants with challenging behaviours and significant people in their lives to develop interim and comprehensive behaviour support plans. We will also provide training to families and carers to implement the plan and provide continuous evaluation as required.
