Support Coordination and Behaviour Support Teams

Chloe Matthews

Team Leader - Support Coordination/Specialist Support Coordination

I have been working in the Disability sector for over 10 years, and really enjoy spending my time supporting people to reach their goals as well as helping them to make sense of the NDIS. I have worked as a mentor, community-based disability support worker, support coordinator, and now team leader, and in all of these roles have taken pride in assisting people to live their lives how they want.

Georgina Noble

I completed university in 2021, where I completed a Bachelor of Arts, specialising in Gender studies.
(Educate and identifying the injustice and challenges in social and cultural norms, advanced knowledge of social, political and cultural workings of gender etc).

Whilst completing my degree, I worked in case management with the Department for Child Protection and the Department of Human Services (Disability SA).

Working at Child Protection gave me a very good understanding of the policies and procedures in which DCP have in place, whilst there I undertook a number of trainings based around trauma, which has helped greatly in the role of SC.

Strengths:
– Working and understanding with the LGBTQI+ community
– Psychosocial disability
– Working with children

Eunice Xue

I am Eunice Xue, holding a Master’s degree of social work from Flinders University. My journey in the field of social work has equipped me with the skills and empathy needed to make a meaningful impact on the lives of individuals facing diverse challenges. I have always worked with people with disabilities since I graduated.

Currently working as a support coordinator and psychosocial recovery coach, I believe in the importance of choice and autonomy. I am dedicated to assisting each person to maximise their potential, achieve their unique goals and receive the best possible support. Whether it’s developing personalized plans, connecting with service providers, or navigating the complexities of the NDIS system, I am here to guide and support you with every step of the way.

Outside of work, I love music, movies, sports, travelling, photographing, and spending time with friends and family.

Fay Fang

Support Coordinator/Specialist Support Coordinator

I have worked with people with disabilities since I graduated from my Master of Social Work degree. I feel like the most rewarding part of my job, is seeing my participants achieve their goals and maximize the use of their NDIS plans, which aligns with my values and job satisfaction. It makes me happy. I am a people person and always have a big smile on my face.

Outside of work, I love traveling, photographing, and spending time with family and pets.

Esther Mackie

Support Coordinator and Psychosocial Recovery Coach

Having initially supported students with disabilities in the education sector, I transitioned to the role of a disability support worker – a position I found deeply rewarding. My commitment to making a meaningful impact led me to become a support coordinator with Affirming Futures, where I continue to gain fulfillment from helping others and contributing positively to their lives.

Ivy Sun

I am grateful to be a part of a team dedicated to improving the quality of life of people with disabilities. I have a background in social work and have worked with NDIS participants for a few years now. I have worked as a disability support worker, and I also have experience in the aged care and mental health field. Now, I am working as Support Coordinator and Psychosocial Recovery Coach to support NDIS participants know the plan better, manage the plan and help them find the suitable service providers.
