What is the NDIS?

The National Disability Insurance Scheme (NDIS) provides individualised support for people with a disability. It will provide all Australians with a permanent and significant disability, aged under 65, with supports deemed reasonable and necessary to live an ordinary life.

  • How can I access the NDIS?

    To be eligible to access the NDIS, a person must meet certain access requirements. These include

    • Living in an area where the NDIS is available
    • Being an Australian citizen, permanent resident or hold Protected Special Category Visa
    • Meet the disability or early intervention requirements
    • Be under 65 when the access request is made

  • My NDIS plan

    Once you have been made eligible for supports – you will receive your individualised NDIS plan.

    Your individualised plan may include supports in the areas of education, employment, independence, accommodation, social participation and health and well-being.

    AFFIRMING FUTURES will discuss your plan in detail with you to assist you to work towards your goals and identify how your funding can be best utilised to meet them.

    These may include

    • support with daily personal activities
    • enable participation in community, social, economic and daily life activities
    • assist with seeking and maintaining employment
    • therapeutic supports
    • support with household tasks
    • arrange for equipment assessment, home modification assessment, mobility equipment

  • Reviewing my plan and my goals

    Affirming Futures will provide individualised support to participants to review your plan and goals. Affirming Futures will assist participants to request plan reviews if additional support is required.

    Affirming Futures commits to supporting participants to work towards your goals at a pace that suits you and with supports that are matched to meet your needs.

  • Choosing my NDIS provider

    Affirming Futures offers a variety of supports including accommodation (long and short term), social support, in home assistance, community support, support coordination and behavioural assessments. If Affirming Futures deems it appropriate to refer to alternative providers – you will be supported to make a choice that best meets your needs.

    Affirming Futures values choice and control for participants and will work with you to get the most from your plan.
